- STUDENT - Fill in student's full name.
- GRADE - Your student's age appropriate grade (while academic skills may reflect various grade levels, holding a student back or skipping a grade requires a conference with the principal).
- TEACHER- Indicate teaching parent or the person designated as teacher at the time of enrollment.
- YEAR - School year usually reflects the year school started and the year of completion (ex. 93-94).
- ATTENDANCE - Attendance is recorded initially on the school's attendance record. At the end of each quarter the summary information from the attendance record is transferred to the report card.
- STARTING DATE - Re-enrolling families, enter the date in which you began schooling during this school year. Some families began schooling during the summer. Others started in September. New families, enter the date of enrollment.
- NUMBER OF DAYS IN ATTENDANCE - California law requires a minimum of 175 days per year, 3 hours minimum of schooling per day. At the end of each quarter, count up the number of days in attendance from the attendance record and indicate that amount in the row under the correct quarter.
- NUMBER OF DAYS ABSENT - While many home schooled students still study on days when they may not feel the best, an absent day is one in which no schooling occurs due to illness or for other reasonable excused absence. SCS requires a notification if absences total more than 5 per quarter. Indicate the absent days from the attendance record.
- TOTAL DAYS - This row is a year-to-date total of schooling days(Add together the days in attendance and days absent).
- HEIGHT - Enter your student's height.
- WEIGHT - Enter your student's weight.
- GRADING KEY - "ABC" grading key is required for junior high and senior high school students. You may utilize the "Outstanding, Satisfactory, etc." grading key for elementary.
- REMARKS EACH QUARTER - You may include notes on how things are going with home schooling, your student, etc.
- CONDUCT - Give an indicator (Outstanding, Satisfactory, etc.) on your student's conduct.
- ACADEMIC SKILLS -Grade the areas that apply. For instance, one might grade all of the Language Arts subcategories (Reading, Phonics, etc.) while another parent might give one grade for the total Language Arts.
Copies of Report Cards are due at the monthly parent meeting following the end of each quarter (consult your school calendar for the end of quarter dates).
